The flow chart describes the working relationship between commissioners and health improvement specialists when constructing a “bespoke” response to a health challenge.
It starts with the commissioner identifying the need for a tailor made response.
The central blue boxes define Commissioner led activity. The gold boxes indicate HIS activity. The green suggest joint commissioner/HIS activity.
The process falls into discrete, sequential parts:
- Commissioner briefs HIS as to the issues and challenges
- Problem/issues identification using, epidemiological, HIA’s and other tools
- Agree strategic response – develop costed option responses for consideration
- Agree costed response and establish governance structure and contractual requirements
- Implement and monitor – feedback to commissioner
