Replacement Certificates
RSPH candidates or centres who have lost their original certificate(s) may apply to the RSPH for replacements by completing the application form available to download here

Please note that this form is not to be used for incorrect or damaged certificates. For incorrect certificates, please return the certificate with the correction required and a new certificate will be issued. For damaged certificates, please return the certificate and a new certificate will be issued.
The RSPH will not be able to provide a replacement certificate unless it is able to verify the original award by reference to its records. The fee will not be refundable even if it has not been possible to issue a replacement.
Replacments have the same standing as the originals that they replace but they are produced in the layout currently used and may not be facsimiles of the originals.
Replacement certificates (single or batches) will be issued FREE if:
- The application is made to the RSPH within three months of the issue of the original certificate(s)
A fee of £30 will be charged for a single replacement certificate if:
- The application is made to the RSPH three months or more after the issue of the original certificate
A fee of £50 will be charged for a batch of replacement certificates if:
- The application is made to the RSPH three months or more after the issue of the original certificates
In all cases replacement certificates will be marked as such.
Certificates are valuable documents and should be stored in a safe place.
