Somerset County Council Public Health's comprehensive approach to system change and innovation activity has seen the launch of both a Homelessness Reduction Board and the Somerset Homeless & Rough Sleeper Nursing Service, where a range of partners collaborate to address the health inequalities experienced by those who are homeless or vulnerably housed, supporting Somerset’s Better Futures programme.
