You & Type 2 is a project designed to reduce health inequalities and improve the consistency of care for people with type 2 diabetes in South London through digital plans. Combining innovative digital technologies and support, the project provides patients with their own personal plan of care, education and support.

The project is built upon contemporary public health policy and frameworks, with a clear focus on the delivery of universal personalised care. The project is innovative and has provided some helpful learning in the use of digital technologies to enhance patient care and improve outcomes.
